Currently have 5 openings available for Case Mangers/Registered Nurse/LPN.
Job Responsibilities:
-Identify and asses members needs
-Plan, coordinate, monitor, and evaluate healthcare services and outcomes through care of clients members
-Meet any health needs of members and provide communication tools between member and provider(s)
This opening is SHORT TERM with the potential to go longer/direct. This opportunity if full time, Monday through Friday from 8:00am to 5:00pm.
